We work closely with Therapy Dog International. We have been thinking about providing "baseball" type cards for the dogs. Do any of you provide anything like this? Or something similar?

We do. We have several facility dogs from Canine Companions and each dog has their own card. It's a great leave behind when they visit with patients and families.
Yes! They are really fun for patients and staff! We partner directly with WAGS based in Louisville, KY. It's a great organization, attached our examples
We have Pet Therapy Trading Cards - marketing takes "head shots" of the pups, and we have stats listing
"favorite toy", "barks at", "favorite napping spot", "breed", "age", and so on.
On the back of the card there is info on how to request a pet therapy visit.
